  • Routine and Emergency First Aid
  • Medication Administration per Physician Orders
    • The parent and physician must complete and sign the Medication Authorization form. 
    • Non-prescription medications (such as Tylenol or Ibuprofen) must also be authorized.
    • Medications must be labeled with student’s name, name of medication, dosage, date, and amount received.
    • Medication Authorization form can be downloaded from this website under “Health Forms”.
  • Student Individual Health Care Plans
    • If your child has an ongoing or temporary medical diagnosis of a health condition, injury or disability which may require medication, limitation, PE restrictions, daily routine health care, emergency first aid, or any other health care support, please contact the school health office.  If applicable, a Health Care Plan may be developed to help insure the health and safety of your student at school. 
  • California State Mandated Health Screenings
    • A school Nurse will check y our child’s vision and hearing as requested by a teacher and through a yearly vision screening in TK, Kindergarten, 2nd and 5th grade.  These tests will/may be administered unless you submit a letter denying consent for your child to be tested for vision and/or hearing.

Immunization Update and Tracking
 
- All students must provide a copy of immunization records.  Students need to be up to date for their age or have a temporary or permanent medical exemption before the first day of school. 
 
Starting  July 1, 2019

Students Admitted at TK/K-12 Need:
  • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses 
    (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.)
    For 7th-12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday. 
  • Polio (OPV or IPV) — 4 doses 
    (3 doses OK if one was given on or after 4th birthday) 
  • Hepatitis B — 3 doses 
    (not required for 7th grade entry) 
  • Measles, Mumps, and Rubella (MMR) — 2 doses 
    (Both given on or after 1st birthday) 
  • Varicella (Chickenpox) — 2 doses
